摘要:
目的 探讨背侧钢板内固定治疗桡骨远端背侧粉碎性骨折的临床疗效。方法 纳入2016年6月至2018年6月期间上海中医药大学附属普陀医院骨科收治的86例桡骨远端背侧粉碎性骨折患者作为研究对象,按随机数字表法将研究对象分为两组(各43例),对照组采用外固定支架固定,观察组采用背侧钢板内固定。观察两组患者治疗效果、疼痛视觉模拟评分(VAS)、Cooney腕关节评分及影像学检查情况(掌倾角、尺偏角、桡骨缩短及关节台阶)。结果 观察组治疗效果优17例,良18例,可6例,差2例,优良率为95.35%; 对照组治疗效果优8例,良16例,可10例,差9例,优良率为79.07%。观察组优良率明显高于对照组,差异有统计学意义(P<0.05)。术前两组患者VAS评分、Cooney腕关节评分及影像学检查情况差异无统计学意义(P>0.05)。术后1年观察组VAS评分为(1.02±0.26)分,对照组VAS评分为(1.85±0.43)分,观察组VAS评分明显低于对照组; 观察组Cooney腕关节评分疼痛、功能状况、活动范围及握力分别为(19.68±2.84)分、(19.46±3.38)分、(19.65±2.96)分及(19.28±3.24)分,对照组分别为(17.74±2.59)分、(17.22±3.14)分、(17.47±2.85)分及(16.99±3.06)分,观察组各项Cooney腕关节评分均高于对照组; 观察组基于X线片测得的掌倾角、尺偏角大于对照组,桡骨缩短及关节台阶长度小于对照组,差异均有统计学意义(P<0.05)。结论 背侧钢板内固定治疗桡骨远端背侧粉碎性骨折效果较佳,腕关节功能改善明显,疼痛缓解,影像学复查效果满意,值得临床应用及推广。
