摘要:
Hill-Sachs损伤(HSL)是肩关节前脱位时造成的肱骨头后上方压缩性骨折,根据关节盂轨迹理论将其分为在轨型HSL和脱轨型HSL。脱轨型HSL保守治疗效果差,通常需要手术治疗,而手术方案选择不当易造成肩关节再次脱位。针对肱骨头或关节盂进行手术将脱轨型HSL转化为在轨型HSL是治疗目的,目前主流治疗方法包括Remplissage术、肱骨头骨移植术、Latarjet术及Eden-Hybinette术等,不同术式各有利弊,应根据关节盂与肱骨头的骨缺损程度制定手术方案。
