摘要:
膝关节单髁置换术是治疗终末期单间室膝关节骨关节炎的有效方法,其围术期并发症发生率低于全膝关节 置换术(TKA)。衬垫脱位是膝关节活动平台单髁置换术后特有的并发症,一旦发生将严重影响临床疗效。近年研究显示, 术后衬垫脱位发生率较低,最多见的为原发性脱位,由假体对线不良、屈伸间隙不平衡、术中内侧副韧带损伤、骨赘 或骨水泥撞击等因素导致;其次为继发性脱位,多由慢性内侧副韧带松弛、股骨假体松动、衬垫磨损及衬垫断裂等所致; 还有一些为外伤引起的创伤性脱位。原发性及继发性衬垫脱位主要与不正确的手术技术相关,严格规范的手术操作 是规避其发生的关键。术后衬垫脱位的治疗依据脱位原因确定,对于有明确可纠正原因的脱位,主要选择原衬垫或 更换衬垫治疗;对于无法确定原因的脱位或术中存在不可纠正因素时,采取翻修为TKA 的治疗。
