摘要:
脊髓型颈椎病可导致颈脊髓功能障碍,常需手术治疗。手术治疗方式分为前路手术、后路手术、前后路联合手术和微创手术。前路手术的优点在于可直接切除颈脊髓前方致压物,减压效果明显,植骨融合率高,可减少生理曲度丢失发生率; 缺点在于颈脊髓前方严重长节段压迫存在减压不充分的问题。后路手术适合多节段颈脊髓重度受压患者,属于间接减压,减压较为充分且安全; 缺点在于可引起颈后轴性痛、C5神经麻痹、颈脊髓神经损伤、颈椎生理曲度难以保持等并发症。颈脊髓前后同时严重压迫的患者通常需要行前后路联合手术。微创手术具有安全、有效、微创的优点,但适应证较严格,应用范围较小。
