摘要:
不可修复性肩袖损伤(IRCT)的治疗一直是运动医学临床工作中的重点和难点。动力性重建通过肌肉肌腱转移来重建损伤的肩袖组织以恢复盂肱关节的力偶平衡,从而缓解疼痛,改善功能,提高力量。对于不可修复性后上肩袖损伤,背阔肌转移是经典选择,大圆肌转移是二线选择,三角肌肌瓣转移是传统方法,下斜方肌转移是较符合解剖结构的新兴方法; 对于不可修复性正后肩袖损伤,背阔肌与大圆肌联合转移是恢复外旋功能的合适选择; 对于不可修复性正前和前上肩袖损伤,胸大肌转移是经典选择,胸小肌转移是二线选择,而背阔肌转移具有广阔的应用前景。该文对不同类型IRCT动力性重建研究进展作一综述。
